2010 Fall Sports Athletic Forms
***** PROCEDURE FOR SUBMITTING ATHLETIC PHYSICAL EXAMINATION PAPERWORK FALL 2010**
Due to recent budget cuts there will be very limited time that staff will be available to collect athletic physical forms.
If your child is having their physical before the end of the school year it would be best to submit it prior to the summer vacation!!!
PLEASE NOTE: ALL HEALTH FORMS MUST BE SUBMITTED IN PERSON TO THE RIDGEWOOD HIGH SCHOOL HEALTH OFFICE . HEALTH OFFICE STAFF WILL BE COLLECTING THESE FORMS PRIOR TO THE LAST DAY OF SCHOOL AND ON AUGUST 3rd & 4th From 10-1 pm AND ON AUGUST 11th & 12th FROM 10-1 pm.
DO NOT MAIL FORMS OR DROP IN MAILBOX , THEY WILL BE RETURNED!!! THE HEALTH OFFICE IS NOT OPEN OR STAFFED DURING THE SUMMER!!!! ALL FORMS MUST BE REVIEWED AND PROCESSED BY THE MEDICAL STAFF AND WILL BE RETURNED TO YOU IF INCOMPLETE. PAPERWORK MUST BE COMPLETE IN ORDER TO BE CLEARED. Every student that submits a physical will be given a letter signed by the school physician stating that they have been cleared which then needs to be given to the coach on the first day of practice!! If the student does not have a letter he/she will be unable to practice!!
IF THE STUDENT CANNOT SUBMIT THE FORMS ON THE SCHEDULED DATES HE/SHE MUST MAKE ARRANGEMENTS FOR SOMEONE ELSE TO SUBMIT THEM FOR HIM/HER ON ONE OF THE ABOVE DATES! **THESE ARE THE ONLY DATES THAT PAPERWORK WILL BE COLLECTED***
*****Fall season sports include: Football, Girls Soccer, Boys Soccer, Girls Tennis, Girls Volleyball, Cross Country, Gymnastics, Cheerleading. (Marching Band participants are also required to submit the athletic packet as per Band Director Mr. Haas)

The NJ Department of Education Annual Athletic Pre-Participation Physical Forms:
Part A: Health History Questionnaire (to be completed by a parent)
Part B: Physical Evaluation Form (completed by the physician conducting the exam)
Every physical form must be complete (this includes measurements for height, weight, blood pressure, pulse rate and vision screening) INCOMPLETE FORMS WILL BE RETURNED!!! If you already have the Athletic Packet on file and the physical exam is dated after August 16th 2009 then you will only need to submit the RHS Seasonal Health History Update and Consent Form. Students will be unable to tryout or participate in any practices unless all forms have been submitted. I have attached the required health forms to this email, you may also go to the RHS website, all the required forms are available to download on the Health Services page, the Athletic packet is also available in the Health Office. Thank you for your cooperation, we realize the athletic requirements are lengthy but the safety and well being of our student athletes is of the utmost importance.
